Connected Care
Our staff at Connected Families are here to support anyone over the age of 13 years with Autism and, or mental health needs. All plans are tailored to meet the needs of each individual and time will be spent carefully matching staff to individuals to ensure the best outcomes are achieved.
We recognise the vital role that families also play in the care of their loved one and how overwhelming at times this can be. With that in mind, we offer a free service to family members who may need support through direct advice or signposting to appropriate agencies.
If you wish to be supported through Connected Families a manager will come out to discuss your needs and complete an initial assessment. Ensuring that both the service user and where appropriate, their family are part of the initial assessment. This allows us to have a holistic overview of each person’s requirements and ensure that appropriate care and support is put in place.
Staff at Connected Families are able to support in the community, within the home, educational settings and hospitals for emergency admissions. They can also support as appropriate adult in custody and support to attend and engage with probation services. This will take place on either a 1-1 or 2-1 basis dependant on need. A clear plan of support will be put in place, with activities that enhance and support each individual's journey.
As part of the Connected Families family, you will also be able to access our community wellbeing activities, which are aimed to support safe socialisation, mental health, wellbeing and positive overall health.
If you have a personal budget, use direct payments or are self funding it can be daunting chosing the right support for yourself or a loved one.
We can help you to plan and organsie your support. Listening to your needs and matching you with the right support workers, We will ensure support is tailored to your needs.
We are currently able to support people within Chorley, South Ribble, Preston and Blackburn with Darwen. If you live outside of these areas, please still contact us as there may have been changes to our demographics.
Connected Families offers regular events to promote socialisation, mental health and wellbeing. Events range from group bowling, country walks, walking charity expeditions to day trips both in the UK and abroad.
Events are separated for young people and adults. All young people ages 13-17 must be accompanied by a parent or carer.
Adults, 18+ may attend events without a carer where it is deemed appropriate. There will be staff and volunteers to support individuals if required and ensure that everyone has fun!
